The Iron Lung

The Iron Lung

An iron lung is a machine that enables a person to breathe when normal muscle control has been lost or the work of breathing exceeds the person's ability. It is a form of medical ventilator. Properly, it is called a negative pressure ventilator.

This Machine founded in the middle of 1900s, Used in manegment of victims of poliomyelitis to help them to survive.

As we know "or we will know", polio virus causes damage in the AHC "Anterior Hron Cells" which are responsible about motor movements of our muscles.
If the AHC of the Respiratofy muscles affected, the patients become unable to breathe

These mesirable patients were placed in these steel chambers to survive.

The iron lung "as we see in the above photo" is a a cylindrical steel drum. A door allowing the head and neck to remain free is then closed, forming a sealed, air-tight compartment enclosing the rest of the person's body. Pumps that control airflow periodically decrease and increase the air pressure within the chamber, and particularly, on the chest.

The small windows in this chamber help in the nursing of the patient


We can see here a pediatric department managing polio cases in the mid of 1900s "polio outbraeks"

When i was preparing this topic, i found in my old files this news about the oldest patient who survived for about 60 years in this iron lung
She died in May 2008 due to Power failure ...!

Hi Dr.Ramzy, 
I heard before that, chickenpox will be eradicated within one year... is this true????

The answer:
This fact is not true at all
because of that:
The virus, after active infection, migrate through the sensory nerves to make Latency in the supplying ganglion.
The formed Abs can't affect the viruses inside the ganglion, also the antiviral drugs
So, you can't guaranty at all  of us don't have the virus in our normal healthy state.

Regarding Smallpox, the situation is different and eradication was successful due to the following factors:

Small pox virus (virola) has a single, stable serotype.

Human is the only host.

There is no carrier state or sub-clinical infection .

The disease is easily recognized clinically.

The antibody response is prompt, so exposed person can be protected.

أنا كان عندى استفسار بخصوص
pathogenesis of HCV
عايزة اعرف ايه اللى بيحصل بالظبط
هو المفروض لما بيتنقل عن طريق الدم بيلف الاول فى الدم لحد مايوصل الكبد ... وخلال الفترة دى بتقدر خلايا الجهاز المناعى انها تستقبله وتهاجمه
like dendritic cells& phagocytes .... which present the antigen to the T helper cells
T helper cells are activated & activate the B cells
then when the virus enters the hepatocyts.....T helper cells activates the CTLs that cause hepatocellular damage & necrosis

ازيك يا دكتورة
سؤالك جميييل جدا جدا
ولازم كلنا نكون فاهمينة كويس اوي
لان الالتهاب الكبدي مشكلة خطيرة جدا في مصر وبنقابلها كل يوم تقريبا
 

اللي انت قلتيه مظبوط
الفيروس بعد ما بيدخل الجسم ويلف في الدم
ال phagocytic cells بتحاول تسيطر
وبتعمل Ag presentation لخلايا المناعه
معني كده ان المناعه بتتنبه ضد فيروس C؟
اه طبعا
وبيتكون اجسام مضادة  Abs
وكمان CMI in the form of specific T cells
امال ايه المشكلة...؟ وليه المناعه دي مش بتقدر عليه..؟
خلاصة الامر بعيدا عن اي تفاصيل ماتهمنيش كطالب في سنه تالته
ان الفيروس لما بيدخل خلايا الكبد بكده يكون هرب من تأثير الاجسام المضادة Abs
وده منطقي لأن الفيروس خلاص بأه intracellular

طب وال CTLs ؟
درها بيجي في المرحلة دي... لأنها شغلانتها كده
CMI
يعني تضرب الخلايا اللي فيها مشكلة
وخلايا الكبد في الحالة دي مليانه مشاكل "الفيروس طبعا"
وعلشان كده تبدأ ال CTLs في الهجوم علي الخلايا المصابه وقتلها
ده بهدف انها توقف تقدم الهجوم الرهيب للفيروس
يعني عايزه تحد من انتشاره
المشكلة الخطيرة هنا ايه
ان الخايا بتاعة الكبد اللي بتموت مش بيطلع مكانها خلايا جديدة
لأ ده بيطلع مكانها الياف

وده سبب حدوث تليف الكبد
تخليلي ان القصه بتحصل كل دقيقة في حياة المريض....!
وده معناه ايه..؟
معناه ان الكبد بيتدمر خليه خلية وجزء جزء
لغاية مايبدء الثمن الاخير منه انه يتأثر
"ماهو من فضل الله الانسان منا ممكن يعيش بحاوالي تمن خلايا الكبد"
وتبدأ هنا الكارثة....
الفشل الكبدي

ارتفاع في ضغط الوريد الكبدي البابي
portal hypertension
وما يليه من قئ دموي ونزيف من دوالي المرئ او الشرج
استسقاء بالبطن وتورم الجسم
